| Reason for the Action |
This market action follows a Product Defect Correction in 2024 (RC-2024-RN-00914-1), following reports of blocked syringe bores to the TGA and Link Medical Products.
The syringe barrel of the product has been updated from a HYPAK syringe barrel to a HYLOK syringe barrel, eliminating the need for adaptors and enabling compatibility with additional connectors.
A recall is being undertaken for legacy HYPAK stock as the use of legacy stock with incompatible connectors or connectors without suitable adaptors may lead to blockages which can delay administration in emergency situations such as the management of cardiac arrest or emergency treatment of acute anaphylactic reactions.
There has been reported deaths associated with blocked syringes. |
| Instructions for Customers/Users |
Customers are to:
1. Inspect your stock for units from the affected batch (0155765).
2. Contact Link to arrange the provision of replacement units and updated A5 trolley card and Safety Letter.
On the provision of the new HYLOK product, customers are to
1. Quarantine units from the affected batch (0155765) to prevent further use.
2. Contact Link to arrange collection of the affected stock for destruction. Alternatively, they may organise destruction of the product, on the condition that a certificate of destruction is provided to Link.
3. Discard any and all legacy material and use the updated A5 trolley card, Safety Letter and Education slides. Staff who handle this product should be trained accordingly on these updated materials.
4. Co-locate compatible connectors with the product where it is stored for emergency use (e.g. emergency trolleys or kits).
Please ensure current stock on hand is not returned or destroyed until you have received replacement stock. |
